Someone explain how this company is no1 on serps in separator box.

I did a keyword check on LTP.
On google when I search, the 1st two options are paid ads as expected.


Then the list of the top 10 are also there (as showed by LTP) and correlated with LTP screenshot.

But in between the paid ads and the top 10 is a picture of related said keyword, and it explains the keyword and linked to one of the business website who are in no 1 position and no 6 position in the LTP screenshot.
The no1 result is in a box, has option to click...
